Summary

This pull request ports opencode-eyesight to OpenCode v2. OpenCode v2 does not run v1 plugin implementations (see OpenCode v2 migration docs), which leaves v0.2.1 inactive on current OpenCode versions.

User / Tool Image
       │
       ▼
[Active model accepts images?]
├── Yes ──> Pass request through untouched
 └── No ──> Check LRU cache (sha256 + model)
├── Hit ──> Inject cached description
└── Miss ──> Describe via isolated vision session
└── Wrap in nonce marker & update request

Why

In OpenCode v1, plugins registered subagents and hooked into v1 configuration structures. OpenCode v2 replaced that model with Plugin.define / setup(ctx) and lifecycle hooks on session (context and compaction). As a result, v0.2.1 does nothing when loaded in OpenCode v2.x. This update restores eyesight functionality on modern OpenCode releases using native v2 APIs.

What changed

  • Plugin architecture: Migrated from v1 config augmentation to OpenCode v2 hooks (ctx.session.hook("context", ...) and ctx.session.hook("compaction", ...)).
  • Vision model integration: Uses any vision-capable OpenCode provider/model directly via short-lived internal sessions, relying on OpenCode's own provider configuration and authentication.
  • Image handling: Replaces inline images (base64, raw bytes, and base64 data: URIs) with text descriptions for text-only active models, both for direct user attachments and tool/MCP results.
  • Request-only substitution: Session history remains unaltered. If a user switches back to a vision-capable active model, original images are passed through.
  • Concurrency & caching: In-memory LRU cache (up to 256 items keyed by image SHA-256 and vision model ID) and a concurrency limiter (up to 4 simultaneous transcriptions). Failed transcriptions are never cached.
  • Repository cleanup: Dropped the v1 @opencode-ai/plugin and @opencode-ai/sdk devDependencies, updated build target to Node ESM, and refreshed the test suite for v2.

Breaking changes

  • OpenCode v2 only: Dropped v1 compatibility and removed the v1 plugin entry point.
  • Configuration and environment variables:
    • The vision model is configured via the model plugin option or the OPENCODE_VISION_MODEL environment variable.
    • v0.2.1 environment variables are removed:
      • OPENCODE_VISION_FALLBACK_MODEL (replaced by OPENCODE_VISION_MODEL)
      • OPENCODE_VISION_FALLBACK_PROMPT (replaced by OPENCODE_VISION_PROMPT)
      • OPENCODE_VISION_FALLBACK_PROMPT_FILE (replaced by OPENCODE_VISION_PROMPT_FILE)
    • Subagent registration in config.agent.vision is no longer used; options are read directly in plugin setup.
    • promptFile now requires an absolute path. A relative path disables the plugin with an error log.

Security notes

  • Tool-free vision session: Vision calls run in isolated internal sessions with explicit deny-all permissions (action: "*", resource: "*", effect: "deny"). Additionally, the plugin strips all tools from the vision session request object.
  • Data minimisation: Only the image and a fixed system prompt are sent to the vision model. Accompanying user message text is not forwarded.
  • Nonce-wrapped boundaries & marker neutralisation: Descriptions are wrapped with a random hex nonce generated when the plugin loads ([Vision model description <nonce>: ...] ... [End vision model description <nonce>]). Text in the description matching marker patterns is neutralised to prevent prompt-injection delimiter escapes.
  • Untrusted data notice: Injects a system notice explicitly instructing the text-only model that vision descriptions are untrusted data from image content and must never be executed as instructions.
  • No fetching: http://, https:// and file:// image URLs are never fetched or read; only inline image data is described. Unsupported protocols, non-image MIME types, and files exceeding 20 MB produce static [image omitted: <reason>] placeholders.
  • Bounded execution: Model capability lookups timeout after 5 seconds, and vision calls timeout after 120 seconds. Failure leaves the request unblocked with an inline transcription notice. No external metadata or credentials are echoed.

Testing

  • Automated tests: 38 tests passing (bun test). TypeScript typecheck (tsc --noEmit) and build (bun run build) are clean.
  • Live verification: Tested on OpenCode v2.0.24:
    • Verified a text-only model successfully read text from an attached screenshot via the plugin.
    • Verified that when the active model was vision-capable, the plugin bypassed transcription and passed requests through untouched.

Open questions

  • Versioning: We left package.json at 0.2.1 so you can decide how to release this change. Because this is a breaking change that drops v1 support and renames configuration keys, we suggest bumping to either 0.3.0 or 1.0.0.

Summary by CodeRabbit

  • New Features
    • Updated the plugin for OpenCode v2. Configure a required vision model from any OpenCode provider, with an optional custom prompt supplied inline or through an absolute-path file.
    • Image-capable models receive requests unchanged. For text-only models, supported images in messages and tool results are replaced with descriptions; requests remain unchanged when image support is unknown.
    • Unsupported images, remote image URLs, oversized images, and description failures receive omission or error placeholders without stopping the request. Vision requests have timeout and concurrency limits, and repeated descriptions can be cached.
  • Documentation
    • Updated setup and usage guidance, including configuration, image limits, timeouts, caching, and session permissions.

V1 plugin implementations do not run on OpenCode V2. This rewrites the plugin
on the V2 API (Plugin.define/setup, session context and compaction hooks).

- Vision model is any OpenCode provider/model that accepts images, set with
the model option (providerID/modelID) or OPENCODE_VISION_MODEL. Provider auth
and endpoints come from OpenCode's own configuration.
- Each image is described in a short-lived internal session with deny-all
permissions; the plugin also removes every tool from that session's request.
Only the image and a fixed prompt are sent; user text is not forwarded.
- Descriptions are wrapped in nonce markers; forged markers in image text are
neutralised and the system notice marks descriptions as untrusted data.
- Remote image URLs are never fetched; unsupported or oversized images become a
fixed omission note. No attacker-controlled metadata is echoed.
- Every external call is time-bounded; unknown model capability leaves the
request unchanged; failures are never cached.
- V1 entry point removed.

📝 Walkthrough

Walkthrough

The plugin now uses the OpenCode v2 API. It resolves vision-model settings, transforms supported images for text-only active models, and registers context and compaction hooks. The changes also update package configuration, documentation, and tests.

Security Architecture Review

Security architecture risk: 🟡 Moderate · up to 459b9

Additional histories containing images and descriptions are now retained rather than cleaned up. The design includes access restrictions and input controls, but isolation after completion and cleanup after interruption still require validation.

Retained concerns

  • Medium · security · inferred: The base attempted to delete each auxiliary vision session after use; the new lifecycle intentionally retains image-bearing sessions and descriptions after success or failure. The 256-entry memory cache does not bound these retained histories, and cache misses can create further histories for images in old messages. This extends sensitive-data retention and makes its lifetime dependent on external retention controls. The behavior is documented, but those controls are not supplied; this is a privacy-lifecycle concern, not a verified disclosure.
Security review details

Security Blast Radius

  • inferred — An attachment author or tool-response source can control image content processed through the configured provider when a known text-only request reaches the hooks. The relevant exposure is image content, derived descriptions, retained auxiliary histories, and provider resources available to the host. The supplied evidence does not establish additional tenant, environment, or administrative reach.

Security Findings and Attack Paths

  • inferred — The supported PR-specific security concern is longer auxiliary-history retention, rather than a demonstrated permission bypass. Image content is explicitly submitted to a session that is no longer deleted by the plugin, extending the lifetime of an additional copy and its description.

Trust Boundaries and Controls

  • observed — The image-loading boundary accepts supported inline representations, rejects remote URLs, and applies a 20 MB decoded-size limit. These controls avoid adding plugin-side remote retrieval and constrain individual images before description.
  • observed — Descriptions cross back into the active request as untrusted text. Nonce-bearing delimiters, marker neutralisation, and a system notice distinguish image-derived content from instructions; the vision prompt also directs transcription rather than execution. These are instruction-level controls, not proof that arbitrary image instructions cannot influence a downstream response.
  • inferred — Tool isolation after successful completion depends on persistent host permission enforcement because the retained session is no longer in the internal-ID set. Local tests establish live-session tool removal, but do not establish production permission behavior or subsequent retained-session requests. This remains an evidence gap rather than an observed escape.

Resilience and Maintainability Implications

  • observed — The four-slot limiter bounds outer description promises, not every unsettled host operation. After bounded interruption, failed descriptions can return while detached cleanup still awaits work; the slot is then released even though the internal-session guard remains. Effective resource containment therefore depends on host cancellation behavior.

Hardening Proposals

  • proposed — Define the retention and access policy for auxiliary image histories. If indefinite retention is unnecessary, use supported expiry or cleanup facilities; otherwise document the additional copies and their ownership explicitly.
  • proposed — Validate persistent deny permissions, retained-session requests, and interruption completion against the supported production host. Where interruption cannot conclusively stop work, consider an outstanding-session budget that remains charged through recovery rather than only through the returned description promise.

Cache hits and duplicates of in-flight images no longer wait for a limiter
slot. README states the actual wait bound instead of 'never blocked'.
